The role involves facilitating referrals to the post-acute homecare/hospice agency identified in the acute care setting, providing education to acute care staff including physicians about available services, and potentially engaging in business development activities related to expansion or new program development.

Minimum Requirements

  • Bachelor’s degree or equivalent experience required.
  • Graduate of an approved clinical education program and current clinical license in the state preferred.
  • Minimum of two (2) years' experience in an acute care setting, customer service, sales/marketing, or relevant experience.
  • Valid Driver’s license and reliable transportation.
  • Commitment to the mission, Organizational Code of Ethics, and adherence to the Compliance Program.
  • Strong oral and written communication skills.
