Job summary

A HUG AWAY, INC is seeking compassionate registered nurses for Home Health and Hospice in the Houston area and surrounding counties. You will perform patient visits, assess needs, and coordinate care with the care team.

Requirements include a Texas RN license, 1 year of hospice/home health with verifiable references, WellSky/Kinnser experience, and on-call availability. A Bachelor's degree is preferred; strong communication and EMR experience are valued. Driving is required.

Qualifications

• Licensed RN in Texas or Compact state.
• Minimum 1 year of hospice and home health with verifiable references.
• Experience with electronic medical records (EMR) desired.
• Home Health or hospice assessments including consents in home, hospital, or facility.
• Driving required to provide patient care.

Responsibilities

• Perform patient visits according to the Plan of Care or patient need.
• Deliver care and instruction aligned with the plan of care.
• Coordinate care with wound care agencies, doctors and team members via Kinnser.
• Perform case management for assigned patients.
• Respond to concerns from home care patients and families.
• Complete comprehensive assessments using OASIS/HIS datasets.
• Attend IDT/case conference biweekly.
