Job summary

Inova Health System's Inova Compounding Pharmacy is seeking a dedicated Clinical Infusion Pharmacist to join our team. This full-time, day-shift role runs Monday through Friday with weekend coverage as needed.

As a Compounding Pharmacist, you'll prepare customized medications (creams, capsules, sterile products), maintain rigorous quality control per USP and , and provide clinically informed counseling to optimize patient outcomes.

Qualifications

• Education: Doctor of Pharmacy (PharmD) or bachelor's degree in pharmacy.
• Licensed Pharmacist in VA.
• Certification: BLS within 90 days of hire.
• Experience: 1 year in compounding or specialty as Clinical Pharmacist.

Responsibilities

• Prepare and compound customized medications (creams, capsules, suspensions, sterile preparations) per physician prescriptions and formulas.
• Select, measure, and mix pharmaceutical ingredients using specialized equipment.
• Maintain quality control following USP <795> and <800> guidelines to meet safety standards.
• Document compounding activities per state board of pharmacy and SOPs.
• Interpret, prepare, and verify medication orders; provide clinical consultation and cost-effective alternatives.
• Handle controlled substances per DEA and VA Board of Pharmacy requirements.
• Participate in formulary review and approvals; monitor therapies for interactions and allergies.
• Report adverse drug reactions promptly; support substitutions and minimize non-formulary procurement.
• Supervise pharmacy support staff; participate in continuous quality improvement.

Clinical Pharmacist Job Responsibilities:

• Prepare and compound customized medications - including creams, capsules, suspensions, and sterile preparations - according to physician prescriptions and established formulas.
• Select, measure, and mix pharmaceutical ingredients using specialized equipment such as balances, ointment mills, and encapsulation machines.
• Maintain strict quality control by following USP <795> and <800> guidelines, ensuring all compounded products meet regulatory and safety standards.
• Document all compounding activities accurately in compliance with state board of pharmacy requirements and company SOPs.
• Interprets, prepares, and verifies medication orders as appropriate.
• Provides clinical consultation and cost-effective alternatives.
• Issues controlled substances in compliance according to the DEA and VA Board of Pharmacy requirements.
• Participates in the medication formulary review and approval process.
• Monitors drug therapies for interactions, allergies, and appropriateness.
• Reports adverse drug reactions promptly and accurately.
• Minimizes non-formulary procurement and supports substitutions.
• Supervises pharmacy support staff.
• Participates in continuous quality improvement.
• May perform additional duties as assigned, including, but not limited to:

  • Participating in decentralized rounding with clinical teams.

  • Responding to emergent bedside alerts and codes (adult and pediatric as designated).

  • Precepting learners as appropriate.

Minimum Qualifications:

• Education:Doctor of Pharmacy (PharmD) or bachelor's degree in pharmacy.
• Licensed Pharmacist in the state of VA.
• Certification:Basic Life Support (BLS) within 90 days of hire.
• Experience: One (1) year experience in compounding and or specialty as a Clinical Pharmacist.
