Trillium Health Resources has a career opening for a Complex Transitional Care Occupational Therapy Assistant (COTA). The COTA is responsible for providing input for Complex Care Coordination, targeting those with chronic, unresolved or complex physical, behavioral health and social determinant needs. This includes providing care planning with foundations in national evidenced-based and informed standards to provide whole person care. The COTA completes required documentation/paperwork/tasks in the Trillium software platforms according to established timelines.
