Complex Case Manager RN - Oncology (Remote)

This role involves managing a panel of members with varying health statuses and clinical needs. The incumbent assesses health management needs, develops care plans, and coordinates with a multidisciplinary team to improve health outcomes. Responsibilities include conducting outreach, addressing barriers to care, and monitoring quality outcomes. The role requires adherence to regulatory standards and documentation requirements.

Essential Responsibilities

1. Oversee a panel of members by assessing needs and referring to appropriate resources.
2. Create and implement care plans with goals and re-assessment schedules.
3. Achieve targeted patient outcomes and document all activities in compliance with standards.
4. Maintain knowledge of and adhere to applicable regulations and standards.
5. Perform other duties as assigned.

Qualifications

Education: Required: High School Diploma/GED; Preferred: Bachelor's in Nursing

Experience: Required: 7 years in clinical, case management, or related fields; Preferred: Experience with diverse populations, CBT, motivational interviewing, or DBT.

Licenses & Certifications: Required: RN license in PA, WV, DE, or NY; Preferred: Oncology experience, Case Management certification.
