Job summary

Mass General Brigham is seeking a Social Worker to perform comprehensive social work assessments and develop integrated treatment plans for inpatients, outpatients, and ED patients. The role emphasizes patient-centered care, collaboration within a multidisciplinary team, and ethical problem-solving.

The position is full-time with hybrid work locations in Harbor Medical Practices and Pembroke/S. Weymouth communities, including remote work days with secure, compliant equipment.

Qualifications

• Master's Degree in Mental Health & Behavioral Medicine or Social Work.
• Driver’s License required.
• LICSW or LMHC required.
• 2+ years clinical experience in a medical setting.
• Case management experience preferred.

Responsibilities

• Provides psychosocial assessments of patients and families with social, emotional, interpersonal and/or environmental issues.
• Formulates biopsychosocial assessment, disposition, and treatment plans.
• Collaborate with other professionals to evaluate patients’ medical or physical condition and to assess client needs as part of the interdisciplinary team.
• Monitor, evaluate, and record client progress according to measurable goals described in the treatment and care plan.
