Qualifications

• Master's Degree in Social Work required.
• LICSW state license required.
• 2-3 years of clinical social work experience for the Social Worker II position; 3-5 years for Senior Social Worker.

Responsibilities

• Provides psychosocial assessments of patients and families with social, emotional, interpersonal and/or environmental issues.
• Formulates biopsychosocial assessment, disposition, and treatment plans.
• Collaborate with other professionals to evaluate patients' medical or physical condition and to assess client needs as part of the interdisciplinary team.
• Monitor, evaluate, and record client progress according to measurable goals described in the treatment and care plan.
