Mass General Brigham Community Physicians, Inc. seeks a Per Diem RN to support care coordination, referrals, and enrollment services across the Newton area.

You will coordinate care plans, educate patients and families, and liaise with providers and payers to facilitate services and authorizations. Qualified candidates have an RN license in MA/NH, an Associate's degree (Bachelor's preferred), and 2+ years in case management, utilization review, or discharge planning.

• If RN does not have NH license, must be willing to obtain. MGB will cover cost.

Job Summary

Serves as a core member of the care team coordinates and supports healthcare within the facility, and coordinates referrals for services outside the clinic. Provides outreach and enrollment services to meet eligibility requirements of the program and surrounding counties. Ensures coordination and planning, including community and family support. Focuses on the development and coordination of community service plans. Partners with service providers, families, and patients to create a plan of healthcare and identify additional service providers.

• Proficient in Microsoft Office and industry related software programs.
• Identifying complex problems and reviewing related information to develop and evaluate options and implement solutions.
• Ability to maintain client and staff confidentiality.
• Understanding of diagnostic criteria for dual conditions and the ability to conceptualize modalities and placement criteria within the continuum of care.
• Knowledge of Healthcare and Managed Care preferred.

Schedule and Work Model: Per Diem. Must be available to work at least two 8-hour days per month. Up to 1 day per week is typically available, but not guaranteed. Remote/work from home with flexibility to work on-site at Newton Wellesley Hospital as needed. Employees must use a stable, secure, and compliant workstation in a quiet environment. Teams video is required and must be accessed using MGB-provided equipment. Remote Type Hybrid. Work Location 2014 Washington Street. Scheduled Weekly Hours 0. Employee Type Per Diem. Work Shift Day (United States of America). Pay Range $28.20 - $68.49/Hourly. Grade 98. TEMP.
