Job summary

Clare Medical of New Jersey seeks an experienced Advanced Provider (NP/PA) to deliver comprehensive primary and palliative care for homebound patients around Philadelphia, PA. The role emphasizes patient-centered care, independent clinical judgment, and coordination with a physician supervisor.

The ideal candidate has 1–2 years in primary care with geriatrics; hospice experience is preferred. You’ll work full-time with a flexible, autonomous schedule and strong clinical support from our in-house

Qualifications

  • Board Certified with PA NP certification in PA.
  • Strong clinical skills to accurately diagnose and treat patients.
  • 1-2 years of primary care experience with geriatric patients; hospice experience preferred.

Responsibilities

  • Provide high quality primary and palliative care to homebound patients in and around Philadelphia, PA.
  • Assess health through physical exams and medical histories; plan care.
  • Communicate effectively with office staff across platforms.
  • Deliver patient-centered care to a panel of patients.
  • Develop impressions, diagnoses, and treatment plans.
  • Consult with patients, families, and caregivers during visits.
  • Order diagnostic testing as needed.
  • Prescribe medications and necessary interventions.

Job description

Purpose:

We are looking for an experienced Advanced Provider (Nurse Practitioner or Physician Assistant) to provide primary care to homebound and complex care patients in the Philadelphia, PAregion on a full-time basis. You will apply medical techniques and principles under the supervision of a physician and you will care for a diverse set of patients with chronic diseases.

What You'll Do:
  • Provide high quality care to primary and palliative patients in and around Philadelphia, PA.

  • Assess patient's health by performing physical examinations; obtaining, updating, and studying medical histories and creating the best plan for care.

  • Effectively communicate with office staff and clinical operations across various internal platforms

  • Provide high quality, patient centered care to a panel of patients

  • Develop impressions, diagnosis, and treatment plans

  • Foster trust and consult with patients, families, and caregivers during visits

  • Order diagnostic testing as medically necessary

  • Prescribe medications supplies and appropriate medically necessary interventions

What You'll Need:
  • Board Certified, with a valid NP or PA certification in the state of Pennsylvania

  • Strong clinical skills with the ability to accurately and confidently treat, assess, and diagnose

  • Positive demeanor and strong people skills

  • Ability to confidently problem solve and operate independently

  • 1-2 years of primary care experience with geriatric patients (hospice experience highly preferred but not required)

Compensation:
  • $105,000- $140,000+ (depending on experience and other incentives) (Full Time)
