Job summary

Chestnut Hill Lodge Health and Rehabilitation Center in Pennsylvania is seeking a dedicated RN Wound Nurse to join our care team. The role is Monday–Friday, 7am–3pm, with focus on assessment, planning, and evaluation of wound care for residents.

You will work with an interdisciplinary team to develop treatment plans, monitor progress, educate residents and families, and maintain accurate documentation. A PA RN license and prior wound care experience are preferred to ensure high-quality outcomes.

Qualifications

• Pennsylvania RN license is required.
• Experience in wound care or a related field is preferred.
• Strong communication and teamwork skills.

Responsibilities

• Assess residents' wound care needs and develop individualized plans
• Implement and monitor wound care interventions, including dressing changes and debridement
• Collaborate with the care team to coordinate services
• Educate residents and families on wound care management
• Maintain accurate records and documentation
