BAART Programs, a BayMark Health Services company, seeks a compassionate Substance Use Counselor to support patients through medically assisted treatment and coping strategies in an outpatient setting.

The role involves individual and group counseling, treatment planning and aftercare development, with a focus on crisis management and improving patient outcomes. A CA counseling certification is required, along with a background screen.

Qualifications

• Certified Counselor in the State of California.
• Experience with chemically dependent individuals preferred.
• Satisfactory drug screen and criminal background check.

Responsibilities

• Provide individual and group counseling services; direct service hours at regularly scheduled intervals; coordinate all services throughout treatment.
• Completes Bio-psychosocial assessment.
• Facilitates, establishes and reviews with each patient the initial treatment plan
• Develop aftercare plans and discharge plans
• Ensure efficient and effective delivery of counseling services to all patients
• Other related duties as determined by supervisor
