Job summary

Glengariff Healthcare Center seeks a Speech Language Pathologist (SLP) to assess patients needing rehabilitation and deliver targeted treatment plans. You will set goals, document progress, and educate families while collaborating with an interdisciplinary team to enhance patient outcomes.

Ideal candidates hold an accredited SLP degree and NY licensure, with experience in long-term or geriatric care and strong documentation skills.

Qualifications

• Must have graduated from an accredited Speech Therapist college or university.
• Must have a valid professional license and registration for state of NY.
• Experience with long-term care and/or geriatric care is preferred but not limited to.
• Well versed with documentation writing
• Evaluations, weekly progress notes, re-certification and discharge documentations.
• Collaborates with all disciplines to provide best patient centered treatment and care.
• Exceptional patient service skills and ability to demonstrate clinical excellence
• Report to work on time; adhere to scheduled hours and demonstrate professionalism at all times.
• Perform all other duties assigned by Director of Rehabilitation
• Compliance with confidentiality, HIPPA and Federal Residents Rights.
• Experience with Case Mix Index and PPS scheduling is preferred however not limited to.

Responsibilities

• Assess referred patients requiring rehabilitation and provide direct patient care.
• Establish and administer treatment programs with specific goals.
• Document and record patient condition and educate patients and families.
• Collaborate with an interdisciplinary team to improve life and function.
• Assist in facility-wide programming, policy, audits, and processes.
