Job summary

Glengariff Rehabilitation and Healthcare Center in New York is seeking a Speech Language Pathologist (SLP) to assess and treat patients in our skilled nursing facility. You will develop and monitor individualized therapy plans, set achievable goals, and document patient progress under physician direction.

Ideal candidates hold an accredited SLP degree, NY license, and experience in long-term care or geriatric settings.

Qualifications

• Must have graduated from an accredited Speech Therapist college or university.
• Must have a valid professional license and registration for state of NY
• Experience with long-term care and/or geriatric care is preferred but not limited to.
• Well versed with documentation writing
• Evaluations, weekly progress notes, re-certification and discharge documentations.
• Collaborates with all disciplines to provide best patient centered treatment and care.
• Exceptional patient service skills and ability to demonstrate clinical excellence
• Report to work on time; adhere to scheduled hours and demonstrate professionalism at all times.
• Perform all other duties assigned by Director of Rehabilitation
• Compliance with confidentiality, HIPPA and Federal Residents Rights.
• Experience with Case Mix Index and PPS scheduling is preferred however not limited to.

Responsibilities

• Assess referred patients requiring rehabilitation and provide direct care.
• Establish and administer a treatment program with goals per patient capacity.
• Document patient condition and educate patients and families.
• Collaborate with interdisciplinary team on facility-wide programming and processes.
