Job summary

TEEMA is seeking a compassionate Personal Care Assistant (PCA) in San Jose, CA to support elderly patients in an adult day healthcare setting. You will help with daily living activities, promote safety and dignity, and document vital signs in EMR, while coordinating with nursing and nutrition teams.

The role requires at least 1 year of relevant experience, CA certifications (HHA/CNA) or equivalent, and strong communication skills. Must have reliable transportation and CA work authorization.

Qualifications

• Minimum 1 year of experience providing personal care to frail, elderly, or medically vulnerable individuals.
• Experience using electronic medical record (EMR) systems.
• Experience assisting patients with activities of daily living (ADLs).
• Knowledge of vital sign measurement, meal service, and patient safety practices.
• Strong organizational, interpersonal, and communication skills.
• Ability to follow supervisory instructions and communicate patient needs effectively.
• Ability to work independently with minimal supervision while collaborating with a team.
• Read, write, and speak English; reliable transportation; CA work authorization.

Responsibilities

• Assist elderly patients with ADLs including bathing, dressing, grooming, and hygiene.
• Provide safe, compassionate care promoting dignity and independence.
• Measure, monitor, and document vital signs in EMR as instructed.
• Observe patients to maintain a safe, supportive environment.
• Communicate changes in patient condition to supervisor or healthcare team.
• Assist with transportation, transfers, and mobility as needed.
• Prepare and serve meals, assist feeding, and support nutrition services.
• Support recreational and engagement activities for patient well-being.
• Maintain care areas, restock supplies, and coordinate with nursing/nutrition teams.
• Ensure privacy and confidentiality; participate in training and meetings.
