Qualifications

• Active RN license in good standing in the state of practice.
• Experience in skilled nursing or long-term care setting preferred but not required.
• Ability to supervise CNAs and collaborate with the IDT.

Responsibilities

• Supervise and support CNAs under the Director of Nursing.
• Collaborate with the interdisciplinary team to assess, plan, implement, and evaluate care plans.
• Ensure baseline and periodic assessments are completed on time.
• Prepare for physician rounds and document concerns for review.
• Initiate referrals based on resident needs and coordinate documentation.
• Perform administrative tasks including medical documentation and reports.
• Deliver nursing care within scope and facility policies.
