Job summary

Qualifications

Responsibilities

• In collaboration with interdisciplinary teams, set goals for discharge or ongoing care needs.
• Perform and document complete patient assessments and maintain care plans.
• Identify learning needs to provide individualized patient teaching.
• Administer medications safely per policy and state practice rules.
• Serve as patient safety advocate and coordinate care across departments.
