Job summary

Axiom Gardens of Flora is seeking a dedicated Registered Nurse (RN) to join our care team on-site. You will provide direct patient care to residents, administer medications, monitor vital signs, and participate in individualized care planning to support autonomy and dignity.

Qualifications

• Must possess active RN license in Illinois.

Responsibilities

• Provide direct patient care to residents, including administering medications, performing vital signs, and providing wound care
• Develop and implement individualized care plans in collaboration with the interdisciplinary team
• Monitor and report changes in resident condition to the nursing team and other healthcare professionals
• Participate in quality improvement initiatives to enhance the overall quality of care
• Collaborate with other departments, including social services and activities, to ensure comprehensive care
• Maintain accurate and up-to-date records of resident care and progress
