Responsibilities

• Provide high-quality patient care to residents in our sub-acute medical care, rehabilitation services, and 24-hour skilled nursing care setting
• Conduct patient assessments, develop and implement individualized care plans, and collaborate with interdisciplinary teams to achieve optimal patient outcomes
• Administer medications, treatments, and therapies as prescribed by physicians and other healthcare professionals
• Monitor patient progress, identify potential issues, and communicate effectively with patients, families, and healthcare teams
• Maintain accurate and timely records of patient care and treatment plans
• Demonstrate a commitment to patient-centered care, respect, dignity, and personal attention to the needs of each individual
