Responsibilities include developing individualized care plans, implementing them effectively, and maintaining resident comfort with current nursing practices.

Qualifications

• Active RN license in state of practice.
• Current CPR certification; must obtain CPR within 90 days of hire.
• License in good standing; no abuse/neglect findings.

Responsibilities

• Assure effective quality nursing care focused on outcomes via nursing process.
• Identify needs and input for discharge planning and follow-up.
• Develop individualized care plans with resident/family and care team.
• Implement care plans consistently with resident-centered outcomes using available resources.
• Collaborate with physicians and health professionals by sharing relevant plan changes.
• Deliver and maintain optimum resident care and comfort with up-to-date nursing practices.
