Job summary

Onyx Health Consulting is seeking a dedicated CNA to provide quality nursing care in a long-term care setting. You will support residents according to individualized care plans, maintain dignity, and respond promptly to call bells and care needs.

You will observe changes in residents and report to licensed staff while helping maintain a safe, clean, and homelike environment. The role emphasizes compassion and adherence to safety protocols.

Responsibilities

• Provide individualized attention to residents to maintain or attain highest practical physical, mental, and psychosocial wellbeing.
• Know the individualized care plan and support residents according to it; share observations with charge nurse or care planning staff.
• Attend to residents' needs including grooming, bathing, oral hygiene, feeding, incontinent care, toileting, mobility, ROM, communication, or other care per plan and scope of practice.
• Maintain comfort, privacy, and dignity; interact with warmth to create a caring environment.
• Understand residents' rights, including freedom from restraints and abuse; uphold these rights at all times.
• Promptly report any incidents or abuse to the charge nurse or administration.
• Keep environment safe, neat, and clean; report lighting or equipment deficiencies.
• Protect residents’ belongings; report missing items and assist in locating them.
• Observe changes in condition or behavior and report to licensed nursing staff.
• Provide skin-care to prevent ulcers and skin damage by turning, repositioning immobile residents and applying moisturizers.
• Answer call bells promptly and courteously.
• Perform duties such as vital signs, weights, applying creams, collecting specimens as assigned by the charge nurse.
• Assist with orienting residents and families to the unit and to the nursing home during admissions and transfers.
• Escort residents to appointments and activities as needed.
• Complete CNA records with proper ADL documentation per department policies.
• Follow safety and infection control procedures including hand washing and gloves.
• Support a homelike atmosphere and participate in QAPI processes.
