Job summary

Qualifications

Responsibilities

• Provide direct care to residents in a warm, respectful environment.
• Assist with bathing, dressing, grooming and other ADLs.
• Monitor and report changes in resident condition.
• Develop and implement personalized care plans.
• Collaborate with team to ensure efficient care delivery.
• Maintain accurate records and reports.
• Provide medication administration and vital sign monitoring.
• Participate in care conferences and plan development.
• Assist with meals, feeding, and hydration.
• Maintain a clean environment with infection control.
• Respond to resident needs in a timely, professional manner.
