Job summary

Qualifications

Responsibilities

• Provide direct care to residents in a warm, welcoming, and respectful environment
• Assist with activities of daily living (ADLs) such as bathing, dressing, and grooming
• Monitor and report changes in resident condition or behavior
• Develop and implement personalized care plans to meet residents’ needs
• Collaborate with team members to ensure smooth delivery of care
• Maintain accurate and up-to-date records and reports
• Provide individualized care including medication administration and vital signs monitoring
• Participate in care conferences and contribute to care plans
