Qualifications

• High school diploma or equivalent.
• Preferable one-year experience in a long term care facility.
• Ability to read and comprehend policy and procedure manuals.
• Ability to effectively present information and respond to questions from managers and employees.

Responsibilities

• Plan and conduct individual, small and large group activities.
• Facilitate communication between employees, residents, families, and external agencies to meet residents' needs.
• Participate in community planning related to facility interests and resident services.
• Develop monthly activity calendars and maintain attendance records.
• Assist in discharge planning and development of activity care plans and assessments.
• Arrange transportation for residents for outings, appointments, or discharge.
• Encourage resident participation in self-initiated activities and provide necessary materials (Braille or audio books as applicable).
• Assist with assessment documentation and quarterly progress notes as directed by the Activity Director.
• Maintain a clean, orderly Activity Department and ensure safety.
