Job summary

Qualifications

Responsibilities

Assure that effective quality nursing care is delivered with residents at the center of care.
Identify needs and input for discharge planning and follow-up throughout length of stay.
Develop individualized care plans in collaboration with residents and inter disciplinary team.
Implement care plans consistently and cost-efficiently with resident-centered outcomes.
Collaborate with physicians and other health care professionals by sharing plan changes.
