Job summary

Ayden Healthcare of Waterville and Wauseon is seeking a compassionate Physical Therapist to evaluate patients, develop individualized treatment plans, and guide rehabilitation to improve mobility and overall health.

The ideal candidate will have knowledge of physiology, anatomy, medical terminology and discharge planning, and will collaborate with the care team, educate patients and families, and document progress. PRN opportunities are available to support our mission of quality care.

Qualifications

• Proven ability to assess patients and develop treatment plans.
• Strong knowledge of anatomy, physiology and medical terminology.
• Experience with discharge planning and coordinating care across teams.

Responsibilities

• Evaluate patients' physical abilities and limitations.
• Develop individualized treatment plans based on patient needs and goals.
• Implement therapeutic exercises and interventions to improve mobility and function.
• Educate patients and families on treatment plans and home exercises.
• Collaborate with healthcare professionals for coordinated care and discharge planning.
• Maintain accurate documentation of progress and outcomes.
• Stay updated on PT practices and techniques.
• Provide home community care as needed.
