Qualifications

• Licensed Physical Therapist in PA in good standing.
• SNF/long-term care experience preferred.
• Knowledge of current treatment practices.
• Familiarity with Net Health Electronic Documentation System preferred.
• Ability to manage patients with different personalities.

Responsibilities

• Evaluate, plan, and administer treatment to residents and patients to relieve pain and restore function.
• Deliver treatment in accordance with the established plan of care.
• Provide clinical support and instruct patients, families, and caregivers.
• Measure and record patient’s motor function, strength, and muscle performance.
• Monitor patient responses and modify treatments as necessary.
• Complete documentation in accordance with company policy and state/federal regulations.
