Job summary

CAMPBELL HALL REHABILITATION CENTER, INC. is looking for a compassionate and motivated Physical Therapist to join its dedicated interdisciplinary rehabilitation team in Campbell Hall, New York. You will evaluate residents' abilities, develop individualized treatment plans, and provide therapeutic interventions focused on improving mobility and quality of life.

The ideal candidate will have a current New York State license and be a graduate of an accredited Physical Therapy program. The position offers health insurance, paid time off, and opportunities for professional growth.

Qualifications

• Current New York State license as a Physical Therapist.
• Graduate of an accredited Physical Therapy program.
• Previous experience in a skilled nursing facility or long-term care setting is preferred.
• Knowledge of Medicare, Medicaid, PDPM, and rehabilitation documentation is preferred.

Responsibilities

• Evaluate residents' physical abilities, limitations, and rehabilitation needs.
• Develop individualized treatment plans based on physician orders.
• Provide therapeutic interventions to improve strength, balance, mobility.
• Educate residents, families, and caregivers on treatment plans and safety.
