Job summary

Community Bridges, Inc. (CBI) is seeking a Peer Support Specialist to join a multidisciplinary behavioral health team in Arizona.

The PSS provides recovery-oriented support to individuals and their families, helps coordinate care, and facilitates transitions to appropriate levels of care with the treatment team. Ideal candidates have a high school diploma or GED, at least six months of recovery experience, and a valid ID, with a Certified Peer Support Specialist certification preferred for

Qualifications

• Highschool diploma or GED required; associate degree or higher preferred.
• Minimum six months of recovery from substance use or mental health disorders required.
• Healthcare experience in crisis or behavioral health is a plus.

Responsibilities

• Support patients and families as part of a multidisciplinary team.
• Assist in care transition and discharge planning with RN, PCT, and BHC.
