Job summary

Esperanza Health Centers is seeking a Peer Recovery Support Specialist to provide case management for patients with substance use disorders and assist in prevention efforts. You’ll act as a resource to patients and health care staff to ensure access to required services.

You’ll maintain a caseload, conduct assessments, and develop recovery plans while coordinating with the MAR program and insurance processes. Bilingual English/Spanish required.

Qualifications

• Must have bilingual English/Spanish communication skills.
• Experience with substance use and mental health care preferred.
• Familiarity with MAR program and insurance concepts helpful.
• Ability to collaborate with a multi-disciplinary care team.

Responsibilities

• Provide case management for patients with substance use disorders.
• Develop individualized recovery plans and relapse prevention strategies.
• Coordinate care with medical providers, BH consultants, and staff.
• Maintain documentation in the electronic health record.
• Engage in community outreach and patient education.
