Job summary

Hebrew SeniorLife is seeking a Patient Care Associate to support residents in daily living, safety, and comfort with a team-driven, home-like care approach. You will assist with activities of daily living, monitor environments, and document care in the electronic medical record while maintaining privacy and dignity.

You will participate in rounds, uphold infection control, and engage residents in enrichment activities, ensuring high-quality care and collaboration with nursing staff across shifts.

Qualifications

• Long term chronic care PCA CNA certification preferred or current enrollment in nursing school, with first clinical experience.
• RSU PCA: CNA certification required in good standing upon hire and ongoing.
• Fluent English skills required – verbal and written
• Computer literacy required
• Flexibility and ability to work independently and collaboratively
• Must complete mandatory HSL/HRC educational requirements and programs

Responsibilities

• Recognize and respond to the needs of the patients and families
• Recognize and communicate changes in the patient’s condition to the nursing team
• Adhere to Safe Patient Handling practices and maintain a no manual lift environment
• Participate in Quality Improvement programs
• Assist patients with personal care as indicated in care plans
• Document pertinent information in the patient’s EMR
• Report accidents and incidents as they occur
• Observe and report patient abuse
• Observe skin integrity and report changes
• Attend daily report team and care planning meetings
• Obtain vital signs and basic procedures as directed
• Follow infection control practices
• Attend ongoing training and classes
• Maintain confidentiality and privacy of patients per HIPPA
• Communicate effectively with peers, patients and families
• Facilitate life enrichment activities and patient engagement
• Maintain cleanliness of patient rooms and common areas
• Prepare, serve, and clean up meals and snacks
• Participate in committee meetings
• Other duties as assigned and trained
