Job summary

The Denver Hospice in Denver, CO is seeking a Palliative Care Navigator to manage scheduling for NP providers, coordinate patient care, and handle administrative tasks for the Palliative Team.

The Navigator collaborates with healthcare professionals, coordinates admissions and discharges, documents care in EMR, communicates for timely outcomes, and contributes to a patient-centered plan while supporting community outreach and quality of care.

Qualifications

• Bachelor's degree preferred; clinical or health-related field is a plus.
• 2–5 years of clinical experience in hospice, palliative care, or a related setting.
• Strong communication, organization, and interpersonal skills.

Responsibilities

• Schedule management for NP providers and patient appointments.
• Coordinate admissions, transfers, and care plans.
• Document patient care activity in EMR.
• Collaborate with Hospice teams and community providers.
• Assist with patient and family communications empathetically.
