Job summary

Qualifications

Responsibilities

• Performs evaluations and develops plans of care to restore function and improve quality of life.
• Evaluates patients timely and documents progress for evaluation, billing, and reporting.
• Advises on adaptive equipment to assist daily activities.
• Collaborates with a multi-disciplinary team to support patient care needs.
• Participates in training and support of team members and students.
• Follows treatment plan and updates plan based on guidelines and discharge criteria.
