Qualifications

• Required: High school diploma or GED, effective communication, and ability to work as part of a healthcare team.
• Ability to perform the physical tasks associated with direct patient care.
• Preferred: hospital/facility experience, CPR/BLS certification, experience caring for older adults.

Responsibilities

• Assist patients with activities of daily living (bathing, dressing, grooming, toileting, mobility).
• Help patients transfer, reposition, and ambulate safely.
• Obtain and document vital signs, weight, intake/output as directed.
• Assist with meals and feeding per care plan.
• Respond to patient needs and call lights promptly.
• Maintain clean, safe, and comfortable patient rooms.
• Observe and report changes in condition to the nurse.
• Follow infection prevention and facility policies.
• Communicate professionally with patients and healthcare team.
