Job summary

Live Well Homecare Agency, LLC seeks a Registered Nurse for our Medical Pulmonary Unit to deliver high-quality, patient-centered care in a homecare setting.

You will assess patients, coordinate care, educate families, and participate in quality initiatives, with night shifts and at least 48 hours every 6 weeks including some weekends.

The ideal candidate has an ADN/BSN and CPR certification, and a commitment to nursing standards and laws.

Qualifications

• Nursing degree (ADN/BSN) from an accredited program.
• CPR certification from the American Heart Association.

Responsibilities

• Assess patients and collect health data involving patients, families, and healthcare providers.
• Coordinate patient care plans to ensure interventions are safe, timely, and appropriate.
• Communicate effectively with patients, families, and team members in person, over the phone, and in writing.
• Provide individualized teaching to patients and families based on thorough assessments.
• Participate in quality improvement initiatives to evaluate nursing practice effectiveness.
• Maintain current knowledge of nursing standards, laws, and regulations.
