• Assist residents with daily living tasks and activities.
• Support the assessment and implementation of individual care plans.
• Maintain accurate daily notes and charts in Nourish system.
• Build professional relationships with residents, families and colleagues.
• Participate in goal setting and outcome planning with residents.
• Promote resident choice and involvement in decisions.
• Represent the service positively and contribute to service improvements.
