Qualifications

• Minimum 1 year of professional caregiving experience in a home care, facility, or healthcare setting.
• Hands-on skills delivering personal care (ADLs) and safe transfers.
• Memory care or dementia care background preferred.
• Valid driver’s license, reliable vehicle, and auto insurance.

Responsibilities

• Provide hands-on personal care and memory care support.
• Assist with daily activities, meals, and mobility.
• Observe, document, and report changes to the care management team.
• Follow the Plan of Care and safety protocols on every shift.
