Responsibilities

• Act as a community liaison between nursing, sales, wellness, and leadership.
• Coordinate care between specialists, therapy, home health, and other services.
• Assist with front desk duties including appointment management and scheduling.
• Monthly outreach with residents/families to assess needs and coordinate care.
• Take vital signs, room patients, and prepare them for visits.
• Coordinate after-visit coordination and complete diagnostic testing.
• Manage quality assurance monitoring.
