Job summary

WellMed, part of the Optum family, seeks a Licensed Vocational Nurse (LVN/LPN) for Home Health per diem in San Antonio, Texas. You will provide direct patient care under RN supervision, follow the care plan, and document accurately to support patient outcomes.

Responsibilities include educating patients and families, coordinating with the Clinical Team Manager and Scheduler, and meeting on-call requirements. Travel within the service area may be required while ensuring high-quality care.

Qualifications

• Graduate of an accredited practical nurse or vocational nursing program.
• Current unrestricted license in the state of Texas as a practical nurse or licensed vocational nurse.
• Current CPR/BLS certification or ability to obtain prior to start of employment.
• 2+ years of clinical experience, preferably home health or medical/surgical setting.
• Knowledge of home care regulatory requirements and standard nursing practices.

Responsibilities

• Provides direct patient care as defined by the plan of care and state Nurse Practice Act.
• Implements the plan of care initiated by the registered nurse and documents accordingly.
• Educates patient and family about diagnosis, plan of care, and medications as needed.
• Coordinates home health services with the team and reports needs to RN/Clinical Supervisor.
• Participates in on-call duties and follows infection control standards.
