ANGIES HOUSE LLC in Houston, TX seeks a Licensed Vocational Nurse (LVN) on a contract basis to provide nursing care and health oversight for individuals with IDD in group home and day program settings. You will work under an RN and in collaboration with the interdisciplinary team to ensure high-quality, person-centered care and regulatory compliance.

The LVN will administer medications, monitor health status, perform routine assessments, and maintain thorough documentation.

Qualifications

• Current Texas LVN license in good standing.
• Experience in IDD, group home, or long-term care settings preferred.
• Strong documentation, organization, and communication skills.
• Ability to work independently and as part of a team in a fast-paced, compliance-driven environment.
• CPR and First Aid certification required.

Responsabilités

• Administer prescribed medications and treatments in accordance with DON and physician orders.
• Monitor and document clients’ health status, reporting changes promptly to the RN, physician, and case manager.
• Perform routine health assessments, including vitals, skin checks, and chronic condition monitoring.
• Ensure timely completion of nursing documentation (MARs, progress notes, incident reports, etc.).
• Maintain compliance with HCS, HHSC, and state nursing regulations.
• Assist with infection control measures, health and safety protocols, and emergency preparedness.
• Participate in audits, surveys, and quality assurance reviews, ensuring readiness and accurate documentation.
• Provide health-related training and in-services to DSPs and other staff.
• Educate clients and families on medication adherence, nutrition, hygiene, and health maintenance.
• Support the RN with care planning, IPPs, and client-specific health protocols.
• Work closely with the RN, case managers, program managers, and direct care staff to coordinate holistic care.
• Communicate effectively with guardians, physicians, therapists, and other stakeholders.
• Advocate for clients’ health needs while promoting independence and dignity.
