Job summary

Portwashingtonpa is seeking a licensed vocational nurse to implement the established plan of care for residents in a sub-acute setting. Under RN supervision, you will assist with planning, assessments, medication administration, and treatment delivery to chronically ill and technologically dependent residents.

The role emphasizes compassionate care, accurate documentation, and collaboration with the interdisciplinary team to ensure individualized care.

Qualifications

• LVN graduate from an accredited School of Nursing.
• Current, valid LVN licensure to practice in the state.
• Current BCLS certification.
• Minimum 6 months LVN experience in the past 2 years in a general acute care hospital or equivalent.
• Must be able to read, analyze, and interpret common scientific and technical information, and communicate effectively in English.
• Strong communication and documentation skills.

Responsibilities

• Provide total nursing care for chronically ill and technologically dependent residents.
• Administer medications and perform treatments as directed by physicians.
• Monitor residents and report condition changes to RN, physicians, family and nursing leadership.
• Develop and implement individualized care plans in collaboration with the RN and interdisciplinary team.
• Maintain accurate documentation and comply with facility policies and regulatory guidelines.
• Provide care in emergency situations and follow established procedures.
