Job summary

Heritage Skilled Care is seeking an Licensed Social Worker (LSW) to provide professional social work services for residents in a long‑term care setting. The incumbent will support residents’ emotional and psychosocial needs while ensuring CMS and state compliance.

The role includes conducting comprehensive assessments, care planning with the interdisciplinary team, and documenting progress in accordance with regulations.

Qualifications

• Bachelor’s degree in Social Work (BSW) required.
• Current state licensure as a Licensed Social Worker (LSW) in good standing.
• 1–2 years of healthcare or long‑term care experience preferred.

Responsibilities

• Perform psychosocial assessments upon admission and periodically.
• Develop and update individualized care plans with the interdisciplinary team.
• Document assessments and progress notes per regulatory standards.
• Provide counseling and emotional support to residents and families.
• Assist with discharge planning and referrals to community services.
• Advocate for residents’ rights and ensure regulatory compliance.
