Job summary

Trilogy Health Services is seeking a Licensed Practical Nurse (LPN) to provide direct clinical care and supervise nursing assistants on the designated unit at Ashford Place Health Campus in Shelbyville, IN.

You will perform daily resident assessments, monitor changes in condition, administer medications within scope of practice, and complete required documentation while coordinating with the nursing team to ensure quality resident care and safety.

Qualifications

• Must have and maintain a current, valid state LPN license.
• Current, valid CPR certification required.

Responsibilities

• Visit residents daily to assess physical and emotional status; notify charge nurse and physicians of changes.
• Identify problems and emergencies and initiate lifesaving measures when physician is unavailable.
• Provide holistic care within LPN scope.
• Monitor medications, supplies, and equipment; ensure adequate stock.
• Complete medical forms, reports, evaluations, and charting as needed.
• Report exposures to blood or infectious materials per policy.
