Job summary

Sapphire Health Services in Tigard, OR is seeking a dedicated Licensed Practical Nurse (LPN) to join our skilled nursing facility. The LPN will deliver quality patient care, administer meds and treatments, and ensure patient comfort and safety in compliance with state regulations and facility policies.

The role calls for strong communication, teamwork with CNAs and interdisciplinary teams, and ongoing education to stay current with best practices. BLS and an active OR license are required.

Qualifications

• Current LPN license in Oregon.
• BLS certification required.
• Experience in a skilled nursing facility preferred.

Responsibilities

• Assist with new admissions and provide direct nursing care, including medications and treatments.
• Monitor residents' health, vital signs, and respond to emergent needs.
• Communicate resident needs to providers and coordinate with the care team.
• Document care accurately in EMRs or paper charts and ensure infection control.
