Trail Creek Place LLC in Michigan City, IN is seeking a Part Time Licensed Practical Nurse (LPN) to support wellness functions, document care in electronic health records, and coordinate with residents, families, physicians, pharmacy, and staff. The role focuses on monitoring health metrics and assisting the care team with resident concerns.

The ideal candidate will have a state LPN license, CPR and First Aid certifications, and at least 2 years of experience with seniors.

Qualifications

• High School diploma or GED required.
• Current state LPN license.
• Current CPR and First Aid certifications.
• At least 2 years of experience working with seniors.
• Ability to communicate effectively with residents, families, and staff.

Responsabilités

• Record residents' vital signs (BP, temp, weight, etc.).
• Support ADLs for residents and the care team.
• Manage medication orders and documentation per regulations; follow up with physicians/pharmacy.
• Develop relationships with third-party providers to ensure necessary medical services.
• Educate residents and families in healthcare procedures.
• Document staff training requirements and deliver training as needed.
• Assist residents with ADLs, including medication management, dietary needs, and personal hygiene.
• Order and verify medications from the pharmacy; conduct quarterly medication audits as needed.
• Maintain stock supplies for the care department and ensure a clean, orderly environment.
