Job summary

Heritage Woods of Canton seeks a compassionate Licensed Practical Nurse (LPN) to oversee medication distribution per physician orders and ensure resident safety at all times.

You will develop personalized care plans, foster communication among physicians, staff, residents, and families, and participate in quality improvement initiatives while engaging in ongoing professional development.

Qualifications

• Current state licensure as Licensed Practical Nurse (LPN).
• Current CPR certification.
• Exceptional communication and interpersonal skills.
• Must demonstrate the ability to serve with love, compassion, and dignity.

Responsibilities

• Oversees medication distribution as stated on physician orders, ensuring strict adherence to Community medication management documentation and procedure, and demonstrating steadfast dedication to resident safety.
• Creates personalized service plans tailored to individual care needs while championing resident independence and dignity.
• Cultivates communication between physicians, healthcare providers, and family, fostering an effective exchange of information pertaining to residents’ physical, mental, and social well‑being.
• Contributes toward quality assurance and performance improvement initiatives.
• Engages in continuous learning and development opportunities through attendance at educational seminars, staff meetings, and other learning experiences.
