Job summary

Byron Wellness Community in Fort Wayne, IN seeks a Licensed Practical Nurse to provide direct nursing care and coordinate resident care across the community.

You'll administer medications, monitor effectiveness, and collaborate with physicians, pharmacies, therapists, and the interdisciplinary team to update care plans and ensure high-quality, resident-centered care. Full-time and part-time shifts available; weekend requirements apply.

Qualifications

• Must possess, at minimum, a Nursing Degree from an accredited college or university, or be a graduate of an approved RN/LPN program. Must have an active license.

Responsibilities

• Provide direct nursing care and coordinate resident care throughout the community.
• Administer medications and monitor effectiveness.
• Coordinate care with physicians, pharmacies, therapists, and interdisciplinary teams.
• Update and implement resident care plans; complete required documentation.
• Supervise and support nursing assistants and medication aides.
• Assist with admissions, transfers, and discharges.
• Maintain a safe, respectful, resident-centered environment.
