Job summary

The Center at Park West in Pueblo, CO is seeking an LPN to provide direct nursing care to residents on site. You will administer medications, monitor vital signs, and assist in care plan development to ensure high-quality resident outcomes.

Qualifications

• Current, unencumbered LPN license is required.
• Ability to make independent decisions and exercise sound judgment.
• Strong communication and interpersonal skills with residents, families, and staff.
• Knowledge of nursing practices, procedures, and long-term care regulations.
• Ability to work effectively in a team and supervise others as needed.

Responsibilities

• Provide direct nursing care to residents, including medication administration, wound care, and vital sign monitoring.
• Observe, assess, and report changes in resident condition to physicians and the nursing team.
• Assist in developing and modifying resident care plans.
• Document resident care accurately in electronic health records.
• Collaborate with physicians, nursing staff, and other departments to ensure comprehensive resident care.
• Participate in facility surveys and quality improvement initiatives.
• Maintain a safe and therapeutic environment for residents.
