Job summary

Twin Fountains Home for Nursing and Rehabilitation in LaGrange, Georgia seeks a compassionate Licensed Practical Nurse to join our care team. You will provide direct patient care, administer medications, monitor vital signs, and help with care plans to support residents’ physical and emotional well-being.

Qualifications

• Current licensure as an LPN in the state of Georgia.
• High school diploma or equivalent; an Associate's degree in nursing preferred.
• Minimum 0 years of experience in long-term care; hospital/rehabilitation a plus.
• BLS certification and CPR training.

Responsibilities

• Provide direct patient care, including medications, wound care, vital signs.
• Develop and implement care plans with interdisciplinary team.
• Perform injections, catheter care, and routine procedures.
• Document patient charts accurately and maintain confidentiality.
• Maintain a safe and welcoming environment for residents and staff.
• Participate in quality improvement to enhance outcomes.
