Job summary

Kadima Health and Nursing at Pottstown is seeking a Licensed Practical Nurse to join our team. You will provide nursing services to residents in a rehab-focused setting with compassionate, resident-centered care.

Under the supervision of RNs and physicians, you will administer medications, assist with activities of daily living, and help develop individualized care plans. PA LPN licensure and at least 1 year in long-term care or rehab are required; shift is 3p-11p.

Qualifications

• Current LPN licensure in Pennsylvania.
• Minimum 1 year of experience in long-term care or rehabilitation setting.
• Ability to work effectively in a fast-paced environment.
• Strong communication and interpersonal skills.

Responsibilities

• Provide direct patient care under the supervision of registered nurses and physicians.
• Assist with daily living activities such as bathing, dressing, and grooming.
• Administer medications and treatments as prescribed by physicians.
• Develop and implement individualized care plans in collaboration with interdisciplinary team members.
• Monitor and report changes in patient condition to healthcare team members.
