Job summary

Lebanon Center for Rehabilitation and Healing in Lebanon, TN is seeking licensed practical nurses (LPNs) to join our clinical team. We offer full-time, part-time, and per diem opportunities with competitive pay and benefits.

The ideal candidate will provide direct resident care, administer medications, and collaborate with RNs and physicians in a fast-paced, team-focused setting. A Tennessee LPN license and strong communication skills are required.

Qualifications

• Current LPN license in the state of Tennessee.
• Experience in skilled nursing, rehabilitation, or long-term care preferred.
• Strong clinical and communication skills.

Responsibilities

• Administer medications and treatments as prescribed by physicians.
• Monitor residents’ conditions and report changes to the nursing team.
• Provide direct nursing care and support to residents.
• Maintain accurate and timely documentation in medical records.
• Collaborate with RNs, physicians, and interdisciplinary team members.
• Assist with admissions, discharges, and resident assessments.
• Ensure compliance with facility policies and healthcare regulations.
• Promote a safe, clean, and comfortable environment for residents.
